The fund supports people to have greater choice over their social care and support – with many choosing to pay for Personal Assistants (PAs) to assist them in leading more active, independent lives.

The Transition Fund, is a one-off grant for young disabled people aged 16 to 25 years of up to £4,000 to help them become more independent and actively participate in their local community. Transition Fund | ILF Scotland
