• Your home address - so that we can send your PANS ID Badge to you
• Your employer's name & their preferred email address (with their permission to share with PANS) - for confirmation of your employment status
• Employer’s contact phone number (with their permission to share with PANS) used only if they are uncontactable by email. All of the Employer’s contact details are deleted once verification is received
• A clear passport-style photo of your head and shoulders - for your ID Badge
• If you are revisiting the ‘Join’ page to complete the remainder of the form please provide your PANS Membership number, so you can gain access
